Ralph Fontanez, BSN, RN
Founder of Holy City Healthcare Consultants
Ralph brings over 15 years in the medical field. After nearly eight years as a U.S. Army medic with the 173rd and 82nd Airborne, including a deployment to Afghanistan, he moved into nursing following a medical retirement, working through critical care, hematology and oncology, and infusion therapy.
What he saw next shaped everything that followed. Healthcare was moving into the home through telehealth, wearables, AI, and remote patient monitoring, but every piece of it arrived through a screen or a sensor. The technology was extraordinary and the experience was hollow. So he kept the tools and added back what they were missing: a clinician, in the room, hands on. He calls them Clinical Concierges. They guide, they treat, and they carry a provider plan across the last mile into the home, the place where behavior actually changes and outcomes are actually won.
Today he leads a team that blends traditional bedside nursing with telehealth, wearable monitoring, and remote diagnostics. The mission is straightforward: increase health literacy, reduce preventable hospitalizations, and bring expert concierge care directly to Charleston homes.
